Provides a guide to thorough questioning
regarding the Infertility Brief History for the couple, and an individual
questioning guide for the female and her partner. The Initial Infertility history documentation template offers default capabilities to the most
common answers by the patient for quick professional charting. It has an
introduction paragraph, thus saving time for typing in the subjective matter
in charting the chief complaint.

Complete outline documentation template for the Endocrine Work-up
Appointment Schedule. Simply fill in the dates of all the testing and other
appointments as outlined in the module. Once printed out the patient can use
this as a reference guide for their upcoming tests and follow-up
appointments. This work-up is structured to provide a thorough way to
uncover any co-existing factors in a short duration of time, and not prolong
the frustrating process for the patient. You can easily evaluate the areas
of concern once all testing is complete. This allows you to be comprehensive
in formulating your treatment plan.

Detailed "Fill In" viewing field for all completed Infertility
Work-up Results, your diagnosis and plan for the individual patient. This
section is completed by a staff member. Once completed, you will always be
able to have a quick reference field for easy review of all the results of
tests ordered.

Intra-Uterine Insemination/Artificial Insemination Donor Document or IUI
template offers quick and thorough documentation of the quality; count, pre
and post wash numbers of the specimen received for insemination. In
addition, it includes a professional closing paragraph outlining the
procedure and patient tolerance of the procedure.

Dual Function Template Modules used for either GYN procedures or Infertility
procedures such as the Endometrial Biopsies, Saline Sonograms, and the
GYN-Ultrasound. All provide a thorough, quick charting structure, and closing
paragraphs for limited typing needs, including outlining the procedure and
patient tolerance of the procedure. These documents can be created easily
and are professionally structured.

There are 3 Informed Consents included in the module;
Pregnancy Risks
associated with Elevated BMI, Intra-Uterine Insemination, and
Controlled
Ovarian Hyperstimulation consents. These are scanned in the letter section
of your EMR system, and can be printed out for the patient to sign. In some
cases, it can be signed electronically and then printed out for the
patient's copy. These will automatically become part of the patient's
electronic medical chart.
